All dogs and cats 4 months of age and older must be vaccinated annually for rabies with an anti-rabies vaccine approved by the Texas Department of Health and administered by a veterinarian licensed by the State of Texas.

Proof of Vaccination
Metal Certificate
A metal certificate of vaccination must be securely attached to a collar or harness that must be worn by the dog or cat at all times. The certificate should contain the following information:
  • Certificate number
  • Name, address, and telephone number of the vaccination veterinarian
  • Year of vaccination
Paper Certificate
In addition to the metal certificate (rabies tag), the official rabies vaccination certificate issued by the vaccinating veterinarian shall be maintained by the owner of the dog or cat. It should contain the information designated by the Texas Department of Health, including (but not limited to):
  • Date vaccinated
  • Identification of the animal
    • Age (3 months to 12 months, 12 months or older)
    • Predominant breed and colors
    • Sex
    • Size (pounds)
    • Species
  • Rabies tag number
  • Owner's name, address, and telephone number
  • Vaccine used (producer, expiration date, and serial number)
  • Veterinarian's signature and license number
